Efficacy of intra-articular mesenchymal cell therapy for pain control in patients with Kellgren and Lawrence grade I-III knee osteoarthrosis: a systematic review

ABSTRACT
Introduction: knee osteoarthrosis is the most common pathology related to chronic damage due to local inflammation and wear. Intra-articular treatment reduces symptoms and delays the progression of the disease.
Objective: to determine the efficacy of intra-articular injection of mesenchymal cells compared to placebo for pain control in patients with grade I, II and III knee osteoarthrosis.
Material and methods: a systematic search was conducted in PubMed, Web of Science, CINAHL and ScienceDirect, for randomized clinical trials between 2011 and 2022, evaluating the efficacy of intra-articular mesenchymal cell-based treatments for knee osteoarthrosis. Inclusion criteria were reviewed publications compared to placebo, addressing the efficacy of intrarticular mesenchymal cells using the WOMAC and/or EVA scales, as well as the GRADE High Quality Score.
Results: a total of 6 randomized clinical trials evaluating the efficacy of mesenchymal cells as an intra-articular analgesic therapy were obtained, the results obtained indicate, that this therapy manages to improve pain perception, by decreasing the EVA score and improving the WOMAC score, in a period of 3 to 12 months, being statistically significant with a p value < 0.05.
Conclusions: intra-articular therapies with mesenchymal cells, have demonstrated, a positive effect on pain control and functional capacity for up to one year, as well as their efficacy and safety compared to placebo.
