ABSTRACT
Limberg flap or best known as rombic flap (rhomboidal) is a useful
therapeutic tool in dermatology reconstruction. This flap has
multiples applications to reconstruct primary cutaneous defects
with zones of residual cutaneous perilesional laxity. The rhombic
flap (Limberg) is an irrigated random cutaneous pattern flap with
opponent obtuse angles and straight angles of transposition useful
in cutaneous wide defects. The laxity in the perilesional residual
defect is especially important to guide the transposition vector.
The knowledge of the aesthetic units, its interfaces and the adherence
zones determined by the retention ligaments, are keystones
in the cutaneous reconstruction of the face with the rombic flap.
Closing of primary defects can vary according with the versatility
of the transposition of rhombic flap with even four varieties of
rotation. The variations of transposition in different type of rhombic,
circular, quadrilateral, hexagonal defects; as well as the actual
modifications of the rhombic flap must be considered in the planning
of complex cutaneous reconstruction. The understanding of
biomechanics in the design and the transposition of the rhombic
flap are paramount to prevent postoperative complications. The
rhombic flap is a useful and safe complementary therapeutic tool
in secondary defects due to cutaneous resections in surgical dermatology.
