Inflammatory infiltrate description of scalp blunt wounds: study in 32 biopsies

ABSTRACT
Background: the macroscopic characteristics of a wound to determine
its temporality are non-specific. Therefore, it is necessary to
apply a more reliable method to determine the age of the wound in
humans. In the forensic area, in Mexico there are none microscopic
studies on the histological evaluation of wounds in humans.
Objective: to identify microscopic changes in histological sections
of wounds produced by application of kinetic energy (contusion)
in scalp, as well as its natural evolution in relation to time.
Material and methods: observational, descriptive, cross-sectional,
and prospective study of subjects with blunt wounds located
in the scalp that were collected in the period between December
2018 and May 2019, in a hospital in Mexico City and studied at
the Institute of Forensic Sciences of Mexico City (Incifo).
Results: of 32 samples taken, 29 corresponded to males (90%)
and the remaining three (10%) were females. The time between
the injury and the biopsy (also called age of the lesion), was between
30 minutes to 24 hours, with an average of 207.51 ± 6.9/4
hours. The age ranged from 19 to 85 years. The contusion objects
were diverse, being: the pavement, with 12 cases (40%), stones in
five cases (16.6%), pistol butts with four cases (13.33%); wood bat
and metal tubes in two cases (6.6%). In 30 biopsies, neutrophils
were identified in the interstitial space, and two biopsies were ruled
out as the individuals were taking hypoglycemic agents. 15 cases
(50%) presented a mild inflammatory infiltrate (+), in eight cases
(26.6%) moderate inflammatory infiltrate (++) was observed, and in
seven cases (23.3%) severe inflammatory infiltrate (+++).
Conclusions: the severity of the infiltrate is affected by the biological
variability and strength of the object with which the blunt
wound occurred.
