Analysis of the sintomatogenic zone in a case of musicogenic epilepsy

ABSTRACT
Objective: To describe the emotional component that triggers seizures, based on semiology and neurophysiological studies of a percussionist patient with musicogenic epilepsy.
Clinical case: We report a case of a female patient, who is percussionist of an orchestra and suffered from had musicogenic epilepsy. The trigger for her seizures was certain music that generated in her an emotional state of discomfort and produced seizures. This epilepsy greatly affected her quality of life and was drug resistant. She therefore underwent a left temporal lobectomy that allowed her to control seizures and to continue her work in the orchestra.
Conclusions: The characteristics of this case suggest this relationship between emotion and musicogenic epilepsy due to the type of stimulus that triggers the seizures and the structural and functional compromise reported in neurophysiological and neuroimaging studies. This emotional component could contribute to the treatment of this epilepsy. Furthermore, this case shows that the mesial symptomatogenic zone of the temporal lobe is probably related to the emotional aspects of music-induced seizures, which can be of great value in the evaluation of the epileptogenic zone.
