Stepedotomy results performed by resident physicians using the Amsterdam hearing evaluation plots (AHEPs)

ABSTRACT
Objective: To evaluate the hearing improvement by means of its pre and postsurgery
average, as well as to assess the decrease of the gap in patients undergoing stapedotomy
by resident physicians using the Amsterdam hearing evaluation plots.
Materials and Methods: A descriptive, cross-sectional, retrospective study was
done with records of stapedotomy surgeries performed by 4th year residents of Regional
Hospital Dr. Valentin Gomez Farias, Jalisco, Mexico, from January 2015 to December
2019. The presence of complications and percentage of closure of the postoperative
gap equal to or lesser than 20 dB were analyzed, considering the frequencies of 0.5,
1, 2 and 4 kHz using the Amsterdam hearing evaluation plots (AHEP).
Results: There were analyzed 72 surgeries. The affected ear gap was 30.06 ± 9.80
prior to surgery, operated ear gap was of 15.51 ± 10.69118 (p = 0.011). Success of
75% of the surgeries with closure of the gap at 20 dB was obtained with AHEP graphs,
12% of the surgeries with worsening of bone conduction.
Conclusions: In patients with otosclerosis, stapedotomy is a good surgical option
to achieve a closure in gap of 20 dB. Efforts to maximize experience and improve
outcomes include close supervision, use of a single stapedotomy technique, ongoing
training of residents, and increasing the number of surgeries.
