Non-vascularized bone grafts for reconstruction of mandibular bone defects. Systematic review

ABSTRACT
The reconstruction of bone continuity defects are a great challenge for surgeons due to the shape and biomechanical functions of the jaw, one treatment is the use of non-vascularized bone grafts, but there is no defined criterion on ethology, donor area, size of the defect, complications, and postsurgical controls. The purpose of this study was to evaluate non-vascularized bone grafts for the reconstruction of mandibular bone defects through a systematic review. Following a PRISMA search matrix based on a PICO question. A systematic search was carried out in search engines PubMed, Embase, ScienceDirect, Cochrane, Google Scholar, and manual search in scientific journals between 1948 and 2020, publications in English or Spanish. 29 articles were selected obtaining 1,176 patients treated with non-vascularized bone grafts for the reconstruction of mandibular bone defects. The iliac crest was the most used bone graft, representing 931 (79.16%) patients. Immediate reconstruction was reported in 585 (49.74%) cases, the extraoral approach was used in 581 (55.01%) patients. 550 (67.15%) patients had defects less than 6 cm in length. Success was reported in 993 (84.43%), with a total of complications in 400 (34.01%) patients, infection being the most reported. Because of this, the non-vascularized bone graft for the reconstruction of mandibular bone defects is a useful alternative.
