State of the art in transluminar endovascular treatment of abdominal aortic aneurysm

ABSTRACT
Transluminal endovascular repair of the vascular pathology is emerging as a leading therapeutic modality for aneurysms and aortic dissections. The concept was first created by Dotter in 1969 and JC Parodi published the first clinical experience in patients with abdominal aortic aneurysms treated by endovascular approach with stent-graft. At thoracic aortic level, the applications include a range of surgical high risk patients with aneurysms, dissections and traumatic lesions. Technical success is more than 82%, mortality of 7.5% and an incidence of paraplegia around 3%. Indications in aortic dissection are limited to the relief of life-threatening complications such as unstable type B dissection, malperfusion syndrome, imminent aortic rupture and select cases of stable type B dissection. Treatment goals include the sealed of primary intimal tear, induction of thrombosis of the false lumen, and remodeling of the aorta. Intervention of abdominal aortic aneurysms (AAA) is recommended for high risk patients with suitable anatomy and AAA expanded to › 5.5 cm, enlarged › 1 cm in one year, or became symptomatic. Aortic aneurysms that involved either ascending and descending thoracic aorta constitute the next challenge in the treatment of the major vascular pathology.
