Usefulness of the measurements of the vascular pedicle for the diagnosis of AlDS

ABSTRACT
Objective: To determine the usefulness of the measurements of vascular pedicle width (VPW) made by students and physicians in training for the differential diagnosis between acute pulmonary edema (APE) and acute respiratory distress syndrome (ARDS).
Patients and methods: Seventeen non expert observators-four non certified physicians (NCP) six non ICU residents (NRICU) and 7 ICU residents (RICU)- analyzed 16 portable simple chest X-ray of patients with known diagnosis (nine with APE of cardiac origin and 16 with ARDS). Kappa index were used to estimate the difference intraobservator/interobservator and the groups of observators. P ‹ 0.05 were considered significant.
Results: two hundred seventy-two simple and measurements were made Kappa index was –0.02 (P ‹ 0.16) and 0.21 (P = 0.01) for simple observation and measurement of vascular pedicle, respectively. Kappa index was–0.12 and 0, 0.04 and 0.23, and 0.02 and 0.27 for the simple observation and VPW measurements of X-chest ray, respectively for NCP, NRICU and RICU.
Conclusion: VPW measurements made by non certified physician is not useful for the diagnosis of ARDS.
