Ureterocystoplasty vs enterocystoplasty. Comparative study

ABSTRACT
A comparative, retrospective study was performed at the “Hospital de Pediatría Nacional de Occidente del IMSS” (Mexican Institute of Social Security) in patients with vesical augmentation using as material the ureter vs the intestine, a total of 30 patients, 15 of each group were included, from January 1994 to November 1999; clinical and urodynamic results were compared. Creatinine levels were 2.7 mg/dL and 3.6 mg/dL (P > 0.05) in the pre and postoperative in the uretero-cystoplasty group, and 0.7 mg/dL and 1.2 mg/dL (P < 0.05) in the other group. 74% of the patients augmentated with ureter had previous surgery of the urinary apparatus, and 47% of the other group. The postoperative vesical capacity improved in both groups (P < 0.05), being those augmentated by ureter an average of 246 mL and those augmentated with intestine 331 mL (P < 0.05). The reflux level was grade II for the right kidney in both groups and of grades V and III for the left kidney in the ureter and intestine, respectively. It was resolved or improved in the follow-up; all patients were continent and for the vesical emptying required intermittent clean catheterization, and only 4 patients (26%) achieved spontaneous miction. 3 cases of lithiasis and 9 cases (P < 0.05) of pyelonephritis in the group augmentated with intestine were documented; all patients improved in cystometry. We can conclude that augmentation with ureter has good results, is versatile and with less morbidity.
