El ligamento anterolateral: nuevo enfoque sobre la estabilidad de la rodilla

RESUMEN
A pesar del esfuerzo para el perfeccionamiento de las técnicas de reconstrucción del ligamento cruzado anterior (LCA), se ha reportado que un porcentaje de pacientes continúa con déficit de estabilización de la rodilla, por lo que las estructuras anterolaterales han ganado interés en su estudio acerca de su rol sobre el control rotacional. Algunos estudios describen la existencia de una banda fibrosa independiente llamada ligamento anterolateral (ALL), el cual ha causado controversia actualmente sobre su existencia, anatomía exacta y función sobre su rol de estabilización de la rodilla. Se ha observado que el déficit y/o lesión del ALL ocasiona una inestabilidad rotacional interna de la tibia al incrementar el ángulo de flexión de la rodilla, por lo que un adecuado diagnóstico y tratamiento del ALL en conjunto con el LCA provee una mejora en la estabilización de la rodilla y previene posibles fallos de la reconstrucción del LCA. Este artículo resume sus características anatómicas, biomecánicas, su diagnóstico clínico y por imagen, así como las posibles técnicas en su reconstrucción descritas.
