Hipotermia perioperatoria: su impacto en temblor postoperatorio. Revisión sistemática y metanálisis

RESUMEN
Objetivo: Se presenta un metaanálisis y revisión sistemática para evaluar la efectividad de líquidos tibios versus a temperatura ambiente para mantener temperatura perioperatoria, reducir la estancia hospitalaria y prevenir hipotermia y temblor postoperatorio.
Metodología: Se realizó una búsqueda sistemática (en PubMed, ISI Web of Science y Scopus) por investigadores independientes para identificar ensayos clínicos aleatorizados que compararan temperatura central, descenso de temperatura durante la cirugía, estancia hospitalaria, incidencia de hipotermia y temblores en pacientes que recibieron líquidos tibios versus a temperatura ambiente.
Resultados: De un total de 778 estudios identificados inicialmente, 15 estudios fueron incluidos en el análisis final. La diferencia estandarizada de medias para temperatura central fue 1.396 (p ‹ 0.001), para descenso de temperatura -2.380 (p ‹ 0.001) y para estancia hospitalaria -0.957 (p ‹ 0.001); mientras que la razón de riesgo para hipotermia fue 0.333 (IC 95% 0.164-0.679, p = 0.002) y para temblores postoperatorios 0.499 (IC 95% 0.310-0.805, p = 0.004), en todos los casos a favor de la administración de LT.
Conclusiones: El uso de líquidos tibios se asoció con mayor temperatura central y menor descenso de temperatura al final de la cirugía, así como con menor estancia hospitalaria, menor riesgo de hipotermia y temblores postoperatorios.
