Artritis reumatoide y riesgo de fracturas osteoporóticas

RESUMEN
La artritis reumatoide (AR) es una enfermedad autoinmune crónica, de etiología multifactorial con presencia de diferentes autoanticuerpos, caracterizada por inflamación poliarticular simétrica de más de tres grupos de articulaciones, sean éstas pequeñas o grandes, causa destrucción en el cartílago articular con incremento de riesgo de osteoporosis y fracturas osteoporóticas. La osteoporosis es una disminución de la masa ósea con aumento de la resorción ósea y disminución de la formación que conlleva a la fragilidad de la microarquitectura del hueso. Las fracturas osteoporóticas son 1.5 veces más comunes en pacientes con AR en comparación con controles sin la enfermedad. Su patogénesis es multifactorial con factores genéticos y epidemiológicos relacionados con la enfermedad y tratamientos, principalmente el uso indiscriminado de glucocorticoides, entre otros. Con relación a fracturas vertebrales, éstas se observan de dos a tres veces más que en población general. Esta revisión aborda los aspectos epidemiológicos, el diagnóstico y el tratamiento de fracturas osteoporóticas.
