Medición ambulatoria de la tensión arterial. Su utilidad en la clínica

RESUMEN
Los errores inherentes a la medición auscultatoria, el retiro de esfigmomanómetros de mercurio en algunos países, aunado al desarrollo y mayor utilización de dispositivos electrónicos para medir la tensión arterial, han propiciado el desarrollo de otras técnicas de medición para el diagnóstico y seguimiento de pacientes con hipertensión arterial. La medición de la tensión arterial con dispositivos automáticos permite su registro fuera del consultorio, en el hogar o sitio de trabajo de los pacientes. Este método se llama medición ambulatoria y está representado por dos técnicas: la automedición y el monitoreo ambulatorio de 24 horas. Los dos procedimientos de medición proporcionan registros más precisos y confiables de la tensión arterial y demuestran mayor asociación con la existencia y desarrollo del daño a órganos blanco, así como en la predicción de mortalidad cardiovascular. Desde hace casi 10 años, diversos organismos internacionales han diseñado y publicado recomendaciones para su aplicación en el ambiente clínico, sus principales indicaciones son la hipertensión de bata blanca y la evaluación del efecto del tratamiento antihipertensivo. En esta revisión se describen las características más importantes de ambas técnicas y su utilidad en la evaluación del individuo con tensión arterial alta, que persistentemente se registra con la técnica auscultatoria.
