Transposición corregida de las grandes arterias (TCGA), problema no resuelto

RESUMEN
La TCGA es una de las más fascinantes entidades de las cardiopatías congénitas. La condición de corregida es solo
aparente. Con el paso del tiempo la mayoría de los enfermos desarrollarán falla ventricular sistémica, aun cuando no tengan
lesiones asociadas. Con los estudios de imagen actuales se obtienen imágenes precisas en cada caso, sin embargo el tema
del tratamiento aún no está resuelto. En los casos sin lesiones asociadas y asintomáticos es importante el seguimiento
focalizado a medir la función ventricular sistémica y el grado de insuficiencia tricuspídea. Mientras la insuficiencia sea leve
y la función normal, no parece razonable intervenir quirúrgicamente. En los pacientes con lesiones asociadas significativas
está indicado el tratamiento quirúrgico. El abordaje tradicional no puede evitar la insuficiencia tricuspídea y la falla ventricular
sistémica a largo plazo. La corrección anatómica es la alternativa propuesta para aliviar la carga al ventrículo derecho (VD)
y reestablecer la función sistémica del ventrículo izquierdo (VI), sin embargo es una operación de duración prolongada y
no exenta de riesgo y complicaciones tardías. En esta revisión se abordan los aspectos clínicos, diagnósticos y terapéuticos
a la luz de la bibliografía más reciente y significativa.
