Guía de práctica clínica de disfonía basada en evidencia

RESUMEN
La disfonía se manifiesta con alteraciones en la producción o calidad de
la voz, tono o volumen que altera la comunicación y que tiene implicaciones
en la calidad de vida. Está presente en múltiples afecciones que
pueden ser intrínsecas a la laringe o manifestaciones de enfermedades
sistémicas; por tanto, se trata de un síntoma con un amplio espectro de
causas que repercute de manera importante en la vida laboral y social
del paciente, que amerita el adecuado abordaje sistemático para ofrecer
el tratamiento óptimo. En México no existe una guía de práctica clínica
que, basada en la evidencia médica mejor sustentada, oriente en las
maniobras diagnósticas o terapéuticas a fin de evitar pasos innecesarios,
costosos o que impliquen pérdida de tiempo para el tratamiento
integral del paciente que la padece, por lo que se plantea esta guía
de práctica clínica de disfonía basada en recomendaciones según la
calidad de la evidencia de la bibliografía médica tomando en cuenta
el sistema GRADE para emitir esas recomendaciones. Se analizan los
pasos fundamentales en la historia clínica para lograr la sistematización
diagnóstica y terapéutica del paciente con disfonía. En general, al
implementar la guía se observa una reducción importante de tiempo
desde que el paciente llega a valoración inicial hasta que recibe el
tratamiento adecuado y específico al problema de base que le ocasiona
la disfonía, se evitan pasos innecesarios diagnósticos y terapéuticos y se
reduce la prescripción y abuso de medicación empírica injustificada.
