Pseudoaneurisma de arteria hipogástrica con perforación de sacro y absceso glúteo: reporte de un caso

RESUMEN
Los pseudoaneurismas de la arteria hipogástrica son raros, generalmente secundarios a trauma penetrante o contuso. Representan
menos de 1% de todos los aneurismas y generalmente se presentan en la arteria glútea superior o inferior. Se reporta el caso de un
paciente masculino de 25 años con antecedente de lesión por arma de fuego en región glútea derecha, el cual desarrolló cinco
años después un absceso glúteo posterior a un traumatismo contuso; se realizaron estudios radiológicos en los cuales se observó
destrucción del hueso sacro y en la arteriografía un falso aneurisma de la arteria hipogástrica derecha. Su tratamiento consistió en
ligadura de la arteria hipogástrica, desbridación y lavado quirúrgico. El tratamiento puede ser quirúrgico o con técnicas de invasión
mínima como la embolización durante la arteriografía. El aneurisma puede ser tratado con ligadura de la arteria hipogástrica,
lo cual es una alternativa en situaciones de urgencia.
