Reparación laparoscópica de hernias ventrales: hacia una estandarización de la técnica

RESUMEN
La reparación laparoscópica de hernias ventrales, incisionales, ha reportado resultados tanto equivalentes como superiores en comparación con el abordaje abierto con respecto a recurrencia, complicaciones generales y de la herida, estancia hospitalaria y dolor postoperatorio. Desde su primera descripción en 1990 hasta la fecha ha experimentado diversos desarrollos en cuanto a su técnica quirúrgica, materiales protésicos empleados y el uso de nuevos dispositivos de fijación y sutura laparoscópica. Destaca la tendencia creciente a insistir en el cierre del defecto músculo aponeurótico de la hernia previo al implante intraperitoneal de la prótesis. Esta medida logra una mejor reparación tanto anatómica como funcional, reduciendo los riesgos de seroma y recurrencia. El presente trabajo describe los diferentes pasos de una técnica que al momento podemos considerar estandarizable, haciendo mención de los materiales, suturas e instrumentos de cierre y fijación con los que se cuenta en la actualidad soportados por la mejor evidencia científica disponible.
