Beneficios clínicos de la terapia de reemplazo renal continua en el paciente críticamente enfermo con falla renal aguda

RESUMEN
Introducción: El tratamiento de la insuficiencia renal aguda (IRA) incluye sustitución de la función en modo continuo o intermitente.
Objetivo: Determinar el comportamiento de la función renal y hemodinamia durante la terapia de reemplazo renal continua (TRRC).
Diseño: Prospectivo, experimental de series temporales de un grupo.
Sitio: Terapias Intensivas de tercer nivel en Monterrey, NL, México.
Pacientes: De 18-80 años con IRA en estadio I (Lesión) o F (Falla) por criterios de RIFLE y con enfermedad renal crónica agudizada (IRCA).
Intervenciones: Mediante catéter Mahurker, conexión a prisma en modo hemodiafiltración venovenosa continua, efluente 35 mL/kg (Ronco), tasa de extracción conforme al balance hídrico, líquido dializante/líquido de reinyección de 70:30, mínimo de tratamiento 72 horas. Se registraron datos hemodinámicos y exámenes de función renal.
Resultados: Se analizaron 24 pacientes, edad 45.8 años (22-77); 2 en estadio I (8.8%), 17 en F (60.2%) y 5 en IRCA (21%), APACHE II 23.4 (DE ± 3.1). Mediante test de Friedman hubo diferencia significativa desde las primeras 24 h para urea, creatinina, ph, bicarbonato, frecuencia cardiaca y presión arterial. Mortalidad 70.8%.
Conclusiones: La TRRC normaliza los azoados y el equilibrio ácido-base manteniendo estabilidad hemodinámica desde las primeras 8 horas de tratamiento.
