Seroprevalencia de Helicobacter pylori en pacientes con gastritis crónica de una zona urbana del Estado de México

RESUMEN
El propósito de este estudio fue determinar los niveles séricos de IgG anti-
Helicobacter pylori y la presencia de la bacteria y de sus genotipos citotóxicos en biopsias gástricas de pacientes con gastritis crónica. Se cuantificaron los niveles séricos de IgG contra
H. pylori en 138 pacientes adultos con gastritis crónica por el método de ELISA. Las biopsias gástricas fueron cultivadas en microaerofilia. La identificación de
H. pylori se realizó mediante PCR convencional y la de los genotipos citotóxicos con PCR multiplex. El 94.9% de los pacientes presentó anticuerpos contra
H. pylori, dentro de los cuales 32.8% tuvo niveles séricos de 20-60AU/mL, 9.5% niveles de 61-99 AU/mL y 52.6% niveles por arriba de 100 AU/mL. Las biopsias positivas para
H. pylori fueron 62 (45%). Todas las cepas de
H. pylori fueron vacA+, pero sólo 28 (45.2%) fueron cagA+. Los genotipos de las cepas analizadas fue
on
vacAs1/vacAm1 (46.8%)
vacAs1/vacAm2 (19.3%), vacAs2/vacAm1 (25.8%) y vacAs2/vacAm2 (8%). El 29% de las cepas mostraron el genotipo virulento vacAs1/cagA. Estos resultados muestran que los pacientes con gastritis crónica que estudiamos tienen una alta frecuencia de infección por cepas virulentas de H. pylori, ya que dos tercios de ellas (66%) tienen el alelo vacAs1 y aproximadamente un tercio (29%) posee ambos marcadores de virulencia vacAs1 cagA.
