Tratamiento quirúrgico de la deficiencia intrínseca del esfínter uretral: revisión de la literatura y presentación de dos ejemplos

RESUMEN
Existen informes sobre el tratamiento quirúrgico de la incontinencia urinaria de esfuerzo,
desde principios el siglo XX. El principal avance ha sido el criterio de selección de cada
paciente para cada uno de los procedimientos existentes. La cirugía no puede limitarse a la
uretra y a la vejiga, debe tenerse en mente el equilibrio de fuerzas que se ejercen en
la pelvis femenina. Por eso, es necesario realizar una clasificación detallada del prolapso
de órganos pélvicos, cuantificar los defectos de la fascia endopélvica y realizar un diagnóstico
preciso del tipo de incontinencia urinaria que se está abordando. El clínico que se
dedica a tratar mujeres con incontinencia urinaria sabe que la curación se aprecia mediante
una valoración subjetiva, más que objetiva. En conclusión, la finalidad del tratamiento es la
de crear una resistencia suficiente de salida para prevenir la pérdida de orina, aceptable
desde el punto de vista social. En este trabajo se realiza una revisión de los diferentes
tratamientos quirúrgicos de la deficiencia intrínseca del esfínter uretral, así como la presentación
de dos ejemplos: uno con la técnica de cabestrillo y otro con la cinta vaginal libre de
tensión en el Instituto Nacional de Perinatología.
