Psicosis no orgánicas y otros trastornos mentales graves en la etapa perinatal

RESUMEN

Se describen varios trastornos mentales graves y la psicosis durante la etapa perinatal. Se hace énfasis en las psicosis puerperales y su relación con los trastornos graves del estado de ánimo (afectivos). Se revisa la epidemiología, fenomenología y diagnóstico diferencial de los diferentes trastornos psicóticos no orgánicos durante el embarazo y el puerperio. El tratamiento con frecuencia incluye observación estrecha, hospitalización y tratamiento con medicamentos psicotrópicos. sobre todo neurolépticos y estabilizadores del estado de ánimo. Se hace hincapié en el riesgo de infanticidio, incluyendo los signos de alto riesgo y la necesidad de intervención temprana. Se describe el tratamiento de la mujer con alteración grave o psicótica en la unidad hospitalaria madre-bebé. En ésta, se puede observar la situación de cerca, evaluar la capacidad de cuidados y ayudarla en la dificil transición a la maternidad. Este procedimiento preserva la relación entre la madre y el lactante.
