Hiperglucemia en el enfermo grave. ¿Epifenómeno o factor de riesgo?

RESUMEN
La hiperglucemia es frecuente en el paciente grave y es secundaria al desequilibrio inmunometabólico y resistencia a la insulina que se manifiesta como parte de la respuesta inflamatoria local y sistémica. En diversas enfermedades atendidas en la unidad de terapia intensiva como: sepsis, episodio vascular cerebral, cardiopatía isquémica, traumatismo craneoencefálico y quemaduras, la hiperglucemia incrementa significativamente la morbilidad y mortalidad. Por lo anterior, el control estricto de la glucosa es parte fundamental de los protocolos de tratamiento.
