Pancreatitis aguda asociada a hepatitis A no fulminante. Informe de un caso

RESUMEN

Objetivo: Informar un caso de pancreatitis aguda asociado a infección por virus de hepatitis A en un hombre joven.
Sede: Hospital de tercer nivel de atención.
Informe del caso: Hombre de 17 años de edad que ingresó al hospital después de 5 días con dolor abdominal ictericia, náusea, vómito y fiebre. No tenía antecedentes de otras causas frecuentes de pancreatitis. En la exploración física se observó ictericia en las escleras y deshidratación leve. El abdomen era doloroso a la palpación, con resistencia muscular e hipersensibilidad de rebote en epigastrio y hepatomegalia leve. Las pruebas de laboratorio, al ingreso; mostró elevación de la bilirrubina directa, de la aspartato aminotransferasa, alanino aminotransferasa, amilasa y de la lipasa sérica. El ultrasonido de abdomen mostró el páncreas aumentado de tamaño y disminución en su ecogenicidad, la vesícula y la vía biliar eran normales. El perfil de lípidos fue normal y la búsqueda de microcristales fue negativa. Se demostró la presencia de anticuerpos IgM para hepatitis A. La puntuación de Ranson fue de 1, mientras que las de APACHE II al ingreso y a las 48 horas fueron de 2 y 3, respectivamente. El evento se resolvió sin complicaciones con tratamiento médico y fue egresado del hospital tres días después.
Conclusión: La pancreatitis aguda aunada a hepatitis viral A es una asociación poco frecuente, con sólo 16 casos publicados en la literatura médica internacional. Hasta el momento no existe evidencia directa de que el virus de la hepatitis A sea una causa de pancreatitis aguda.
