Manejo laparoscópico de la úlcera péptica perforada. Primera experiencia en el Hospital Universitario de Caracas

RESUMEN
Introducción: La perforación de una úlcera péptica representa una situación de urgencia que requiere de una intervención quirúrgica para su tratamiento. Presentamos la primera experiencia del Hospital Universitario de Caracas en el manejo laparoscópico de esta patología.
Material y métodos: Estudio retrospectivo de pacientes intervenidos quirúrgicamente con diagnóstico clínico de perforación gastroduodenal que acudieron al Servicio de Urgencias del Hospital Universitario de Caracas entre noviembre de 2003 y septiembre de 2004.
Resultados: Ocho pacientes de sexo masculino con una edad promedio de 48 años (rango 28 a 74), con úlcera péptica perforada fueron sometidos a cierre primario y colocación de parche de epiplón por vía laparoscópica. El tiempo operatorio fue de 160 minutos (rango 120 a 195). El dolor posoperatorio a las 24 horas fue de 2 en la escala visual análoga. El promedio de estancia intrahospitalaria fue de 7 días (rango 4 a 18). El tiempo para la reincorporación a las actividades cotidianas fue de 13 días (rango 10 a 25). Dos pacientes presentaron complicaciones que ameritaron reintervención. No hubo mortalidad.
Conclusiones: La reparación laparoscópica de la úlcera péptica perforada es segura y eficaz. Ofrece las ventajas de un excelente resultado estético y menor dolor posoperatorio.
