Sarcoma mieloide y sus manifestaciones extramedulares

RESUMEN
Antecedentes: El sarcoma mieloide puede ser la primera manifestación de la leucemia
mieloide aguda. El tumor consiste en blastos mieloides maduros o inmaduros
presentes en un sitio anatómico más allá del hueso. Para diagnosticar el sarcoma
mieloide debe estar presente una masa tumoral que provoque borramiento del tejido
local. Tiene predominio masculino con una relación hombre:mujer de 1.2:1, puede
aparecer a cualquier edad y en cualquier parte del cuerpo. Los sitios más comúnmente
afectados son los ganglios linfáticos, la piel, los tejidos blandos, los huesos, los testículos,
el tubo gastrointestinal y el peritoneo.
Caso clínico: Paciente masculino de 40 años de edad que fue diagnosticado con
sarcoma mieloide durante el abordaje de una tumoración a nivel de T11-T12, en
quien durante el aspirado de biopsia de médula ósea se observó celularidad normal,
con reporte de inmunohistoquímica: MPO+, CD-43+ y CD-117+, con índice de
proliferación del 60%.
Conclusiones: El sarcoma mieloide afecta al 2.5-9.1% de los pacientes con
leucemia mieloide aguda. El mecanismo exacto de la afectación extramedular no se
comprende por completo.
