Mastitis subclínica y subaguda: patologías frecuentemente subdiagnosticadas y de serias consecuencias

RESUMEN
La mayor parte de la información que se tiene acerca de las
mastitis subclínicas y subagudas en la literatura es la que se
observa principalmente en el ganado vacuno. Es probable
que se deba a la importancia en la industria ganadera
y a las pérdidas económicas que estas enfermedades
provocan. Aún cuando hasta el momento no existen
criterios diagnósticos precisos acerca de estos cuadros
clínicos en las mujeres, queda claro que la presentación
clínica y evolución no es semejante a la que se observa en
las mastitis agudas o purulentas. Se ha propuesto que el
dolor crónico al amamantar, las grietas en los pezones y la
baja producción de leche podrían estar relacionadas a estos
cuadros. La leche materna no es un líquido estéril, contiene
microorganismos benéficos para la madre y para el bebé. Se
ha propuesto que la disbiosis podría estar condicionando
la predisposición a cuadros inflamatorios de la glándula
mamaria. Puede ocurrir disbiosis debido a factores externos
como la dieta materna, grasa corporal de la madre, estado
de salud, medicamentos, tipo de nacimiento, genética y el
lugar de nacimiento. El objetivo de este trabajo es analizar
los criterios diagnósticos para mastitis subaguda y mastitis
subclínica en mujeres lactantes reportados en la literatura
médica. Se requieren estudios que permitan definir
criterios diagnósticos e intervenciones terapéuticas para
estas condiciones, ya que son causa frecuente de destete
precoz en las mujeres lactantes.
