El manejo del dolor postoperatorio en obstetricia: ¿Es seguro?

RESUMEN
En nuestro país, se atienden 2 millones de nacimientos anuales; este dato nos indica, que el manejo analgésico en la paciente obstétrica, es proporcionado de forma habitual. Por ello, se ha considerado la analgesia postoperatoria en obstetricia, no se circunscribe únicamente al anestesiólogo y que posiblemente interactué con el binomio materno-fetal. El abordaje analgésico de la paciente obstétrica, ha sido motivo de controversias y discrepancias que han limitado su utilización. Hoy en día, aceptamos que el manejo terapéutico de una mujer con dolor debe iniciarse al momento de ser solicitado por la afectada. Esto ha favorecido el empleo de diversas técnicas analgésicas durante el postoperatorio; sin embargo, poco se sabe sobre los efectos que ocurren en el neonato tras la eliminación de fármacos por leche materna. Por tal motivo, el objetivo de este documento es hacer algunas consideraciones sobre los antecedentes históricos de la analgesia obstétrica, los mecanismos productores del dolor, el empleo de estrategias terapéuticas y de la seguridad farmacológica a la luz de la evidencia literaria.
