Management of distal tibial fractures with mobile transarticular external fixation

ABSTRACT
A comparative, prospective, longitudinal, quasi-experimental study was undertaken to find out if the mobilization of the hinge component of transarticular external fixators improved the course of patients with distal metaphyseal tibial fractures. Ten cases were treated with transarticular external fixation, and were divided into two groups of 5 patients each. The first one underwent mobilization of the external fixator hinged component (Mobile Group) and the second one was kept without mobilization (Stiff Group). Patient course was evaluated with the following four variables: healing time, function, painful sequelae and complications. A data collection form was used and then a statistical analysis with simple frequencies and table crossing was performed. The healing time did not change as a result of the hinged-component mobilization; signs of healing appeared after week 10 in 80% of cases. Ankle function was favorably affected by the hinged-component mobilization of external fixators. The intensity of painful sequelae was analyzed in both groups. Most patients had mild-to-moderate pain that frequently led to minor limitation of activities of daily living. The complications reported consisted of superficial infections and there was one case of vicious union.
