ABSTRACT
Objective: To determine the efficacy of a new respiratory index in order to quantificate the prognosis and severity of respiratory failure (ARF) in cancer patients.
Desing: Clinical and prospective study.
Setting; Medical/Surgical ICU of an oncologic hospital of Mexico City.
Patients: During one year 260 oncologic patients assisted with mechanical ventilation were studied. They were divided in three groups: I) Patients admitted to the ICU after a surgical procedure; II) Patients with ARF of variable etiology; III) patients with acute respiratory distress syndrome (ARDS).
Interventions: None.
Measurements and main results: We made a comparison of a new equation: Lung Injury Index in Cancer, LIIC [paw x G (A-a) O2/1000 + Rx] with APACHE II score and Murray Scale. In logistic regression analysis the predictive capacity was more strongly associated with LIIC (area under ROC = 0.764 than APACHE II score = 0.6986; p ‹ 0.05 and Murray Scale = 0.6077; p ‹ 0.05. LIIC was 1.72 ± 1.03, 5.73 ± 2.74 and 14.08 ± 4.07 for groups I, II and III, respectively (p ‹ 0.05).
Conclusion: LIIC can quantificate the severity of ARF and its predictive value is major than APACHE II score and Murray Scale.
