Othello syndrome in multi-infarct vascular dementia

ABSTRACT
Othello syndrome is a psychiatric disorder with a delusional celotypic idea that appears during multiple disorders with dementia. The clinical, diagnostic and treatment guidelines of this pathology were presented with a case of multiple-infarct vascular dementia. The updated bibliography on the diagnostic criteria, etiology and treatments used was reviewed. An Ecuadorian patient from Guayas province, 68 years old, married, hypertensive, without follow-up and with irregular treatment is presented. Psychological tests showed progressive memory impairment, frequent disorientation in space and time, sustained celotypic delusions with torpid evolution, aggressiveness and celotypic distortion of reality. He was treated with risperidone and quetiapine without improvement. Magnetic resonance imaging (MRI) of the skull showed multiple infarcts. The results of the MRI, a neuropsychological evaluation, as well as the progressive and rapid evolution of celotypic symptoms, pose the diagnosis of Othello syndrome during a dementia due to multiple infarcts.REFERENCES
