Hip status in children with cerebral palsy treated prophylactically with soft tissue surgery

ABSTRACT
Introduction: The management of the children with Cerebral Palsy (CP) is multi-disciplinary and the orthopedic treatment a challenge, due to the heterogeneity of this patient group. Preventive surgery for the management of hip displacement involves approaches designed to treat or prevent early displacement, in whom the joint still maintains congruence but demonstrates radiographic signs of "risk".
Objective: Evaluate retrospectively the pelvis AP projections of children with CP treated by medium adductor myotomy and psoas tenotomy (MAP). Also, assess the radiographic efficacy of this procedure to decrease or avoid spastic hip displacement.
Material and methods: Retrospective, descriptive, cross-sectional, and analytical study. Variety of 90 patients with CP spastic treated with MAP between January 2012 and December 2013. The Reimers Migration Percentage (RMP) were registered presurgical and in the last assessment during 2018, with an overall follow-up of 5.5 years after intervention. They were classified according to the Gross Motor Function Classification System (GMFCS). The RMP were classified as good, with a RMP ≤ 24%, regular 25 to 39% and bad ≥ 40%. A statistical evaluation was also carried out with the Student's t tests for related samples.
Results: The patients intervened by MAP in their last assessment had a good-regular result in up to 89%. In addition, a significant difference in the RMP media was demonstrated before and after the MAP treatment, right p 0.013 and left p 0.003.
Conclusions: The preventive treatment has a significant effect on RMP.
