CD20-positive extranodal NK/T-cell lymphoma nasal type. A report of an infrequent case that can be a source of potential diagnostic error

ABSTRACT
Background: Aberrant CD20 expression in extranodal NK/T-cell lymphoma, nasal
type is an extremely rare event and represents a diagnostic challenge.
Clinical case: A 54-year-old woman with a CD20-positive extranodal NK/T-cell
lymphoma nasal type. Patient presented with a destructive right nasal lesion. The biopsy
showed a polymorphic lymphoid infiltrate with angiocentricity and angiodestruction,
resulting in necrosis, apoptosis, and ulceration. Immunohistochemical analysis demonstrated
diffuse and strong positive staining for CD3ε, CD56, TIA-1 and membranous
CD20, but the neoplastic cells stained negative for other B-cell markers (CD79a and
PAX-5). The tumor cells were focally positive for LMP-1, but the nuclei were diffusely
positive for EBER (Epstein-Barr virus-encoded RNA)
in-situ hybridization.
Conclusions: It is interesting that some lymphomas may express both surface proteins
of T and B-cells, but the underlying mechanism remains unknown.
