Prosthesis-patient mismatch clinical aspects a year after aortic valve replacement

ABSTRACT
A descriptive, prospective study of a series of cases was carried out, which included 27 patients in whom aortic valve replacement was performed, either alone or in conjunction with another variety of cardiac surgery, in the period between June 1, 2014 to June 30, 2015 at the Cardiocentro in Santiago de Cuba, with the objective of estimating the Prosthesis−Patient mismatch one year after the valve replacement was performed and to characterize this phenomenon according to clinical and echocardiographic variables. Mismatch was detected in 16 patients, 59.3% (95% CI: 38.9, 79.6). 13 affected patients presented moderate mismatch, 81.2% and 3 patients; 18.8% a severe disproportion. The mass index had an overall decrease of 94% with respect to the pre-surgical value and in 14 patients; 51.9%; the reduction was significant. The septum and the posterior wall returned to normal ranges in both cases, this regression occurred independently to the degree of prosthesis-patient disproportion. There was a statistically significant association between Patient Prosthesis Mismatch and small prosthesis. The Patient Prosthesis Mismatch seems to have limited consequences in the evolution of patients a year after the aortic valve replacement.
