Bacterial transfusion reaction due to platelet concentrate contaminated with Klebsiella oxytoca: a preventable transfusion complication? A case report

ABSTRACT
Introduction: Transfusion reactions as a result of applying platelets contaminated with bacteria are currently one of the biggest challenges of transfusion of blood products. Klebsiella Oxytoca is the second most common cause of bacteremia per gram negative and since the early 1980s has been recognized as a pathogen with clinical significance.
Case presentation: Female patient 1 year old diagnosed with acute lymphoblastic leukemia L1 FAB pre-B CaLLA Negative with thrombocytopenia, A Rh positive. Multi transfused with no previously reported adverse reactions to transfusion. Transfusion reaction is reported during application of the second platelet concentrate. Platelet concentrate is cultured with a positive result for Klebsiella oxytoca. The septic episode was successfully treated with aminergic support, assisted breathing and broad-spectrum antibiotic.
Conclusion: There are multiple strategies for reducing the risk of contamination related to blood products. Klebsiella oxytoca can cause serious infections, bacteremia and septic shock. Clinical care, sufficient case research and reporting (hemovigilance) as well as the implementation of pathogen growth inhibitors are essential to control emerging infections and prevent future complications.
