Reforming the health sector of Guatemala: an organizational and functional model to transform the health system

ABSTRACT
Introduction: the social needs of the Guatemalan population require the transformation of the health sector due to deficiencies in the current model, dissatisfaction with health care delivery, low levels of health access and coverage, high morbidity and mortality levels, and the need to change the system incorporating something different. Objective: develop a new model to reform the system and accelerate progress towards universal health access, improve the health status of the population, increase citizenship satisfaction and protect them against financial risks. Development: health reforms in Latin America are analyzed to understand success and failure factors, as well as lessons learned that could be contextualized in Guatemala. Different reform frameworks are researched and the Theory of Control Knows is selected. An organizational and functional model to transform the health sector is conceptualized applying changes in two of the five control knobs ─organization and financing. Conclusions: the new elected government for the 2020 - 2023 period must decide whether to strengthen the current model that has resulted in a health sector that fails with a mindset of bureaucrats and receptors, or to embark in the adventure to create a model that works with a mindset of entrepreneurs and clients based on a functions, integration of resources and infrastructure, and pooling of funds model.
