ABSTRACT
Background: The cutis verticis gyrata is a very rare clinical sign, consisting of
thickening and folding of the scalp as grooves and convolutions that simulate the
surface of the brain.
Clinical case: A 28-year-old male patient, black, originally from The Gambia,
subjected to periodic health checks, without any pathological history of interest and
without symptoms of any kind at the time of the medical visit. He attended to the office
after discovering a cutis verticis gyrata in the crown area just after shaving his hair. The
history and initial physical examination data were highly unremarkable and suggestive
of essential primary cutis verticis gyrata.
Conclusion: Any asymptomatic carrier of cutis verticis gyrata on the scalp with a
biopsy lacking pathological elements and with normal analytical and hormonal results
has a high probability of suffering from pachydermoperiostosis.
