ABSTRACT
Background: Transfusion related acute lung injury is a rare complication associated
with transfusions. It is an inflammatory process linked to the recruitment and activation
of neutrophils in the lung, resulting in acute lung injury often confused with overload or
pneumonia. The incidence worldwide is reported close to 0.08%; however, in Mexico
there are no records. There is no specific treatment for transfusion related acute lung
injury, the therapeutic measures are hemodynamic and respiratory support with oxygen
and mechanical ventilation.
Clinical case: A male patient with an atypical evolution of acute pulmonary injury
following ten hours of transfusion with fresh frozen plasma.
Conclusion: Transfusion related acute lung injury is a little frequent complication;
its diagnosis requires high clinical suspicion and to discard affections that can be
manifested with sudden dyspnea.
