Reconstruction of complex leg wounds: three years’ experience

ABSTRACT
Wounds can occur and they can be simple or complex. The lower limb they affect the leg up to 14.6% and the fracture is the main lesion. Due to the anatomical characteristics of the leg, it often receives severe injuries that merit complex reconstruction in order to maintain the functional state of the lower limb. We describe the experience of three years in the reconstruction of complex wounds of the leg in the Central Hospital of the State of Chihuahua. We conducted a retrospective study analyzing electronic records of patients with complex leg injuries assessed by the Plastic Surgery Service in the 2014-2017 period. We studied 42 patients with an average age of 42.2 years; of these there were 37 men (88%) and 5 women (12%), whose wounds occurred more frequently in the distal third of the leg (38.8%), with bone exposure in 87.5% of cases. The management was with skin grafting (33.3%), fasciocutaneous flap (16.6%), muscle flap (28.5%), reverse sural flap (11.9%) and free flap (9.5%). The short-term results showed total integration of the flap and/or graft (78.5%), partial (9.52%) and loss (11.9%). We conclude that the complex wounds of the leg require a multidisciplinary management headed by the plastic surgeon, since the reconstruction of these includes skin grafts and/or microvascular flaps.
