Safe percutaneus cistolithotomy in male patients with morbid obesity

ABSTRACT
Introduction: Suprapubic percutaneous cystolithotomy is a useful and safe therapeutic modality for the
resolution of bladder stones, but its performance in patients with morbid obesity represents a special
surgical challenge.
Objetive: To identify the results of percutaneous suprapubic cystolithotomy in male
patients with complex vesical lithiasis and morbid obesity through the practice of a group of technical
details that, in our opinion, are essential for the safe and successful performance of this therapeutic
modality in this kind of patients.
Methods: A descriptive and retrospective study was performed at the National Center to treat urinary stones in Ameijeiras Hospital from October 2016 to July 2018. The sample consisted of four patients with multiple vesical lithiasis or bigger than 2 cm to whom it was performed a percutaneous cystolithotomy. Variables considered were: age, body mass index, stones size, number of stones, comorbidities, surgical time, the presence or not of post-operative fragments, and complications.
Results: The average age was 64.5 years old. The average body mass index was 46.87 Kg/m2. The stone
size fluctuated in the range of 1.5 cm to 4.5 cm. Within the associated conditions, Diabetes Mellitus was
present in 3 (75%) of the 4 cases. The average surgical time was 86.25 minutes. All the patients were stone
free and without complications.
Conclusions: Percutaneous cystolithotomy with the use of the instruments and the practice of the technical and surgical details recommended is an effective and safe option for the treatment of vesical lithiasis in male patients with morbid obesity.
