Management of the morbidly adherent placenta by the PRIMMA protocol

ABSTRACT
Objective: To evaluate the evolution of patients with morbidly adhered placenta left «in situ» after the obstetric event, in order to evaluate the evolution and compare it with the reported as a means of decrease the maternal morbidity and mortality.
Material and methods: Retrospective, observational and descriptive study of a series of 11 patients of morbidly adhered placenta that were managed in La Clínica de Especialidades de La Mujer from the Secretaría de la Defensa Nacional in México in a period of seven years (2003-2010). There were 11 patients that were managed according to the PRIMMA protocol.
Results: In this series six patients presented complete resorption of the placenta, while of the remaining five patients underwent hysterectomy due to infection 3/11, hemorrhage 1/11 and hemorrhage/infection 1/11. No patient required massive transfusion or maternal deaths occurred.
Conclusions: This protocol is considered an alternative management at all levels of care, where human and material resources (blood bank) are limited. It is put to the consideration of obstetric care institutions as a mean for preventing maternal morbidity and mortality.
