The importance of high-resolution HLA typification in hematopoietic stem cells transplantation

ABSTRACT
Histocompatibility is mainly determined by MHC genes, HLA in man which is located in the short arm of chromosome 6. The immune response against HLA antigens is the major obstacle to organ and hematopoietic stem cells transplantation. Actually, the most important transplantation centers and the cord blood banks select the units of cord blood searching HLA antigen matches typed low to medium resolution level for HLA-A and –B and high resolution level for HLA-DRB1. In contrast, bone marrow transplantation HLA class I -A, -B, and -C and HLA class II -DRB1 and -DQB1 are typed in high resolution level to select the donor to prevent GvHD and graft rejection and obtain an outcome transplant.
