Evaluation of aesthetic results in dorsal decubitus after breast reconstruction

ABSTRACT
Although studies concerning satisfaction with breast reconstruction after mastectomy have proliferated, little information is available concerning this satisfaction when the patient adopts changes of position. It turns out obvious to mention that it is the position of dorsal decubitus the one that the woman adopts for several recreative activities and especially for the sexual activity. The determination of this aspect in the different reconstructions might be used by the plastic surgeon for the advice and decision making on the best option for his patient.
Objective: Determine the degree of patient’s satisfaction with breast reconstruction, in this occasion evaluating her satisfaction with the aesthetic result of her reconstruction, when she adopts the position of dorsal decubitus.
Material and methods: 28 patients that were operated for breast reconstruction, were evaluated by the scales modified of Alderman and Lowery.
Results: The patients who were reconstructed with autologous tissues were most satisfied by their results, exception made by the patients reconstructed with bilateral pedicled transverse rectus abdominis musculocutaneous (TRAM) flaps. Women who were reconstructed with autologous tissues and breast implants also showed high indexes of satisfaction, but not those patients who were reconstructed by implants alone.
Conclusion: The patients most satisfied with her corporal image when they are in dorsal decubitus, are those reconstructed with free TRAM flap, lattisimu dorsi with prosthesis, and finally the reconstructed with pedicled TRAM flap. The patients who better evaluated their results were the patients with mastectomy for breast cancer. In the evaluation done by the surgeon, the best results found in dorsal decubitus, were those reconstructed with autologous tissue alone.
