Not justified indications of plasma at the Hospital General de México. A pilot study

ABSTRACT
The plasma is the blood fraction that contains the clotting factors, albumin and immunoglobulins necessary to correct coagulopathies, however, plasma indications are very specific and most of its demands are not justify. In order to know the more frequent not justified indications of plasma in the Hospital General de Mexico; we registered prospectively in a period of three years all the required plasmas at the Blood Bank during the matutine duty. We authorized those requirements justified with abnormal prothrombin coagulation time (PT): ‹ 60% and considering each case solicitude. From 2874 plasma units required on this period, 290 (10 %) had no justification, being the most demanding services: Oncology and Surgery. The most frequent not justified indication was Surgery, followed by “to improve clotting times” (evens with PT › 60%) without evidence of bleeding, thrombocytopenia and hypoalbuminemia. Almost a third of the not justified solicitudes referred normal clotting times and 13% of them did not count with this study. Plasma demands evidences the limited knowledge of its indications and exposes the patients to innecessary risks.
