Giant Pilomatrixoma in Mennonite Population, Case Report

ABSTRACT
Introduction: pilomatrixoma is a benign adnexal neoplasm
derived from the cells of the hair matrix. Its incidence is one per
thousand skin tumors, without predominance of race, affecting
young ages. First line treatment is surgical removal.
Case report: 18-year-old female from the Orthodox Mennonite
community, consults the plastic surgery service due to a
12 cm tumor, in the posterolateral region of the left forearm,
pedicled, with an irregular surface, friable, easy to bleed with
some ulcerated and necrotic areas, incisional biopsy diagnosis
pilomatrixoma, tomography excluded deep invasion, so total
resection of the lesion was performed and reconstructed with
skin grafts.
Discussion: our patient pilomatrixoma was ulcer-tumoral type,
according to Carvajal, perforating appearance with calcium material,
in the tomography it presented the typical characteristics
with an isolated calcification, however due to the size, it presented
atypical behavior and diagnosis was obtained through
biopsy.
Conclusion: delaying diagnosis and surgical treatment can
cause pilomatrixomas to reach an atypical, even giant sizes. Resection
must take advantage of the null invasion of deep functional
structures, with simple reconstructions of good aesthetic
results.
