Systemic lupus erythematosus and pregnancy

ABSTRACT
Introduction: Pregnancy in women with systemic lupus erythematosus remains a high risk for increased mortality and maternal and fetal morbidity.
Material and Methods: 23 patients admitted to
Hospital General de México «Dr. Eduardo Liceaga» were studied.
Results: 43% required admission to the Gynecoobstetrics Intensive Care Unit. The most common variables at admission were associated thrombocytopenia, proteinuria, preeclampsia, urinary tract infection, anemia and arthralgia. There were no maternal deaths. Perinatal outcomes of patients admitted to the Critical Care Unit showed mortality of 70% with extreme prematurity and growth restriction. In patients without critical illness exacerbation, 69% arrived to term pregnancy.
Discussion and conclusions: In this study, immunological tests were unrelated to maternal and fetal prognosis. However, it is clear that patients with thrombocytopenia, proteinuria and fetal preeclampsia have a worse prognosis, so the rating scales for systemic lupus erythematosus should give a higher importance to these variables, in pregnant patients.
