Quantitative elastography in suspicious breast nodule for malignancy

ABSTRACT
Purpose: Describe the shear wave elasticity imaging in the characterization of mammary nodule.
Materials and methods: An observational, retrospective, transversal design was used. Quantitative elastography was used shear wave elasticity imaging. The criteria for the quantification of the speed of the cut waves in mammary nodules was 3.25 m/s for malignant lesions. Images of the breast nodule were acquired in grayscale, color Doppler and elastography, those findings diagnosed as category
Breast Imaging Reporting and Data System (BI-RADS) 4. The images were stored in the PACS system. Nodule biopsy was performed with ultrasonographic guidance and core needle.
Results: A total of 15 patients were assessed, the findings were organized in a table. The pathological findings of benignity or well differentiated carcinoma were coincident for the category BI-RADS 4A, in the case of BI-RADS 4C it coincided with poorly differentiated carcinomas, in the BI-RADS 4B case, the findings were variable, although always coincided with the speed greater than 3.25 m/s.
Conclusion: Elastography combined with conventional ultrasound could potentially increase the level of confidence with respect to the final evaluation of the breast lesion. It allows to have objective criteria for the categorization of BI-RADS 4 in A, B or C, with this, to guide the radiologist and the clinician regarding diagnostic and therapeutic management. In addition, in the case of BI-RADS 4A, even with elastography, a lower BI-RADS could be categorized if the cutting speed is ‹3.25 m/s.
