Granulomatous lichen aureus. A case report

ABSTRACT
In 1996, Saito et described the granulomatous variant of pigmented purpuric dermatoses. Since then 22 cases have been described, six out of them corresponding to lichen aureus. On the other hand, one case of lichen aureus showing a perianexial, dense and deep lymphocytic infiltrate simulating a lichen striatus was described in 2014. We expose a 25-year-old Caucasian woman presented with golden plaque with purpuric spots in its center. Histopathollogically, there was a superficial lichenoid infilatrate with extravasated erythrocytes and abundant siderophages . In the deep dermis, a dense perianexial lymphocytic infiltrate was evident. Small groups of histiocytes forming small granulomas were present. The immunochemical studies and special stains confirms the diagnosis. Thus, Perl’s stain showed abundant siderophages and CD68 marked the granulomas.
