Blood pressure and heart rate during active orthostasis in subjects with prediabetes

ABSTRACT
Objective: To evaluate the changes in blood pressure and heart rate induced by orthostasis in individuals with prediabetes.
Methods: We studied 27 patients with prediabetes (age: 47.52 ± 9.87 years) and 31 with normal glycaemia (age 46.39 ± 9.00 years), assisted at the National Institute of Endocrinology. Electrophysiological records were completed at the Institute of Neurology and Neurosurgery. Oscillometric blood pressure and heart rate of Colin Pilot polygraphic record system were read in supine position and at the first, third and fifth minutes after active orthostasis. The values registered in both groups were compared by means of variance unifactorial analysis and the test of differences between proportions.
Results: Subjects with prediabetes showed higher systolic blood pressure values than normoglycemic controls in the supine position (124 ± 13 vs. 115 ± 12 mmHg, p= 0.010) in the first minute of orthostasis (129 ± 19 vs. 117 ± 15 mm Hg; p= 0.008). The showed lower percentage of increase in heart rate than controls in the first minute (8.0 % vs. 16.5 %; p= 0.003) and third minute (8.55) vs. 18.36 %, p= 0.007) of orthostasis. There were no significant differences between groups in relation to other parameters of blood pressure and heart rate.
Conclusions: Individuals with prediabetes were able to adequately regulate blood pressure during orthostasis, which suggests normality of peripheral sympathetic vascular function. The alterations observed in the control of heart rate during orthostasis are not conclusive to demonstrate cardio vagal injury, as they could be influenced by higher supine baseline blood pressure recorded in subjects with prediabetes.
