ABSTRACT
Introduction: The giant cell giant lesion, formerly called giant cell granuloma, is an entity of idiopathic origin, two variants are described; aggressive and non-aggressive, based on their biological behavior. The aim of presenting this case is to review current concepts of this entity and to give the importance of the use of the CD34 immunomarker, to establish a conservative treatment and to prevent functional aesthetic sequelae.
Clinical case: An 8-year-old female presented with a condition of two months of evolution, characterized by a bulge in the middle third of the right side of the face, the intraoral examination showed a well-defined volume increase in the right upper quadrant, red color, firm consistency, circular shape and not painful to the palpation, in the orthopantomography a poorly delimited radiolucent image was seen in the upper right quadrant region, related to the right upper canine, the histopathological report concluded with a central lesion of giant cells, the immunomarker CD34 was positive in 2%, the treatment was enucleation and curettage of the lesion, with no recurrence after three years of follow-up.
Conclusion: The central lesion of giant cells is a widely known entity and the therapeutic behavior can be performed conservatively, as long as it can be demonstrated by molecular subtype that it is a non-aggressive variant.
