Metabolic perturbations in orthotopic liver transplantation

ABSTRACT
Introduction. The metabolic alterations in liver transplantation, because of higher incidence and magnitude, they can reach in determinate moments of the surgery, are still been a factor of risk in this proceeding therefore the diagnosis and correction are one of the pillars over which rest the clinical management of those patients. Evaluate the incidence of these intra- operatory metabolic alterations in a group of patients with terminal chronically liver insufficiency who was applied orthotropic liver transplantation.
Methods. It was done a prospective, longitudinal and descriptive study to 50 adult patients who have received a liver transplantation in the period from January 2013 to February 2016 at Medical Surgical Search Center.
Results. During the implementation of the proceeding, some important alterations of the acid-basic balance were presented, glycemia, K+, ionic calcium and high temperature were gaining intensity, culminating most of them in the revascularization of the graft, from that moment on a stage of recovering started simultaneously with the restoration of the hepatic function.
Conclusions. Although during the three stages of the operation severest metabolic alterations were presented, it was during the anhepatic stage and mainly in the reperfusion of the graft where there have acquired the highest intensity, being those moments the most difficult in the anesthetic management of those patients.
